Step 1: Get Qualified Through Insurance
Take the Prescription Certification Form to your Cancer/Dermatologist Doctor.
Provide us with your completed prescription form, copy of your drivers license,
copy of your insurance card (front/back). You can email us this information at: info@alleyesonyouhairstudio.com Our team will verify your eligibility and reach out to you to set your appointment.
Step 2: Pick Your Wig
You are now ready for your consultation. You will select your medical wig. Get the measurements of your head to customize the unit. Once that is complete we will place your order.
Step 3: Receive Your Wig
We will submit a claim on your behalf and let you know when your order is ready to be shipped or picked up.
